A handwritten formula book kept by pharmacist Charles C. Neff, Ph.G. The cover and first page are titled "Formulae"; the first page carries Neff's signature with a Camden, New Jersey address, the credits "Philad. College of Pharmacy '05" and "Brooklyn College of Pharmacy '06", and the date August 1903.

The ruled, numbered pages hold handwritten prescriptions and preparations under headings such as Henry's Dyspepsia Remedy, Corn Cure, Hair Tonic, Cough Syrup, Asthma Powder, Pile Suppository, Insomnia Cure and Neuralgia Cure, along with a section of Antidotes for Common Poisons. Pasted in are dozens of printed pharmacy labels, including Neff's own labels from 1726 Fulton Street, Brooklyn (Neff's Expectorant, cold cream, poison and carbolic acid labels) and labels from other druggists in Brooklyn, Camden, Philadelphia and elsewhere in New York State, along with 1906 trade-press clippings, among them New York State Board examination questions.
